- Insurance Cards
- Medication List
- Reports from any Procedures or Tests associated with your Pain.
- A copy of your Referring Physicians Notes from your last visit.
- A completed Patient Information Packet- PATIENT PACKET
- *We do not need Films- X-rays, MRIs, etc. If needed we will request.*
Workman’s Comp and Motor Vehicle Insurance-
1. I was in a car accident, Do you accept motor vehicle insurance?
Answer: Yes, we have a billing specialist that works in conjunction with your MVA insurance and your lawyer to get your medical claims paid.
2. I was hurt at work and I have a workman’s compensation claim, do you accept workman’s comp claims?
Answer: Yes, However Workman’s Compensation laws are very different from laws regarding medical insurance or MVA insurance. There must be an approval from your workman’s comp insurance in order for us to treat you. If this is not obtained, workman’s comp will deny payment of your claim. We can not bill your medical insurance because they will deny payment sunce this was a work related issue. In the end the claim will become your financial responsibility.
3. How do I get approval from workman’s compensation to come to your office for treatment in your office?
Answer: There is 2 ways; the best way is to have your referring doctor call the WC insurance company with the recommendation for treatment with a pain specialist. Another way is for you to call the company and request that you see a pain specialist. Once the request is made, your adjuster or nurse case manager will check the list of preferred providers and will then assign a doctor from that list, you can ask if we are on that list and if you can see us.
